The term "medical license" seems to have done a bait-and-switch along the way.

Exactly. It now appears to be a license to make money so long as you follow the "standard of care" and prescribe the latest poisons! Who needs a Hippocratic Oath when there's good money to be made?

Oh yeah, Hippocrates wept. But that's where it gets tough. I've been thinking about this post by Dr. Nass and the ensuing discussion and I can't see how honest doctors are supposed to make a living and stay out of the clutches of what passes for the law these days.

Very difficult. For one thing you can't be an employee of some large group and second you have to get away reliance on private health insurance and government medical programs.

Right; and even if the doctor sets up some kind of consultancy that does not require licensing (with the help of a friendly super-lawyer), what about the money? How do you ensure that the doctor gets paid but doesn't get booked for that?

It was Hayek, I think, who said that the way you know you are living under a totalitarian regime is, any simple regular everyday action suddenly entails at least five extra steps that you don't understand, and skipping any of them will land you in jail.
